Be sure to include the following:

Legal Name when Student at MVHS

Date of Birth

Year of graduation or last attendance

Name and address of the school or college

A Photograph of a valid ID w/ Photo

and current phone number in case we need to reach out for verification.

There is no fee for a transcript.
